An audio device hardware error occurs or the browser is denied access to the audio device. In this case, you do not have the permissions to use the audio device. If you are on macOS, go to System Preferences > Security & Privacy > Privacy > Microphone to obtain the permissions.

A video device hardware error occurs or the browser is denied access to the video device. In this case, you do not have the permissions to use the video device. If you are on macOS, go to System Preferences > Security & Privacy > Privacy > Camera to obtain the permissions.

The audio device is disconnected. For example, the microphone is unplugged, or the browser permissions on the microphone are revoked.
